As part of the Primary Care Network (PCN) service delivery, Nuneaton & Bedworth Primary Care Network uses the RIVIAM Social Prescribing Hub to support the delivery and coordination of social prescribing and wellbeing services.
RIVIAM Digital Care will be providing the following web-based platforms Nuneaton & Bedworth Primary Care Network on behalf of our practice:
- Referral Form - a secure channel to refer patients into the relevant Additional Roles Reimbursement Scheme (ARRS) Staff and PCN commissioned services.
- Social Prescribing Hub - a platform for the management of patient's health and care needed that will integrate with the Electronic Patient System (EPS).
- Service Directory - a public facing service platform of local community, voluntary and commercial organisations for patient support.
The referral form enables our GP practice to securely refer patients to the ARRS team & PCN commissioned services and allows relevant information to be shared with the professionals involved in supporting our patient's. This may include information required to assess the referral, arrange support, record outcomes and coordinate onward care.
The RIVIAM Social Prescribing Hub is a secure digital platform which supports communication and information sharing between GP practice and the PCN services. Where appropriate, information relating to the patient's referral and support may be shared with the registered GP practice to support continuity of care.
Nuneaton & Bedworth PCN is the contract holder for the RIVIAM Digital Care services. The patient's registered GP practice remains the data controller for the patient's patient data, subject to the specific processing arrangements applicable to the service.
Information is only accessed and shared where there is a lawful basis to do so and where it is necessary to provide, coordinate or manage the patient's care. Appropriate confidentiality, security and information governance arrangements are in place.
